As a Civil Surgeon, Dr. Shin offers comprehensive service for immigration physicals at the most affordable fee in the DFW area.

General process

​    First visit:

  • Review medical history, vaccination records and perform physical examination.

  • Collect samples for the testings needed. Applicants typically need testing for tuberculosis (QuantiFERON Tb Gold test), Syphilis, Gonorrhea, MMR (mumps, measles, rubella), Varicella (Chickenpox) and Hepatitis B. This varies on the vaccination status and age.

    Second visit:

  • Go over test results.

  • Offer immunizations when necessary. This can also be done at local pharmacy. 

  • The applicant signs the second page of Form I-693 in front of the Civil Surgeon as instructed, and the form will be completed and sealed. You will be given the copy of the entire document.

What to Bring to your Medical Exam

  • Form I-693 (Report of Immigration Medical Examination and Vaccination Record ). Please complete the first page

  • Government-issued photo identification such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

  • Immunization records.

Fee for the Immigration Physical 

  • We charge $350 for 15 years old or older, and $300 for those younger than 15. This fee will cover 2 office visits, all necessary testing, and I-693 completion. This does not include cost for additional vaccines if you choose to receive them at our office. 

  • If the tuberculosis test (QuantiFERON Tb Gold Test) is positive it will be reported to the County Health Department according to the USCIS guidelines. If you choose to receive the treatment for the latent Tb infection at our clinic there will be an additional fee of $100.

How long does it take to get the form completed?

It typically takes 3-5 business days to get all the test results.  When lab results are available the second visit will be scheduled.
