General process
​ First visit:

Review the applicant's medical history and vaccination records.

Samples will be collected for the tests required as part of the immigration medical examination. Applicants typically need testing for tuberculosis (QuantiFERON-TB Gold), syphilis, gonorrhea, MMR immunity (measles, mumps, and rubella), varicella (chickenpox), and hepatitis B.

The specific tests required may vary depending on the applicant's age, vaccination history, and individual circumstances.

Second visit:

Review the test results with the applicant.

Offer any required immunizations if needed. Vaccinations may also be obtained at a local pharmacy or other qualified vaccination provider.

The applicant will sign Page 2 of Form I-693 in the presence of the Civil Surgeon as instructed. The form will then be completed, sealed, and prepared for submission. A copy of the completed Form I-693 and supporting documentation will be provided to the applicant for their records.

What to Bring to your Medical Exam

Form I-693 (Report of Immigration Medical Examination and Vaccination Record ). Please complete the first page.

Government-issued photo identification, such as a valid passport or driver's license.

If you are 14 years old or younger, please bring identification that shows your name, date of birth, place of birth, and your parent's full name. Acceptable forms of identification may include a birth certificate (with an English translation, if applicable) or an affidavit.

How long does it take to get the form completed?
It typically takes 3–4 business days for all test results to become available. Once the results have been received and reviewed, a follow-up appointment will be scheduled to complete Form I-693.
